Falls Prevention and Management Service

What is the service?

This service provides specialist multi-factorial falls assessment, multidisciplinary management, advice and treatment for adult patients with a history of risk of falls within their place of residence.  This includes nursing and residential homes within the boundaries of North Somerset.

Who is the Service for?

Adult patients who require professional specialist falls prevention/management in a community setting.
