Stroke Care Co-ordinator

What is the service?

The Stroke Care Co-ordinator provides a community based specialist stroke service with:

  • Patient centred holistic assessments, advice, information and support to self care.Referral to and liaison with other relevant services.
  • Stroke education and training support to colleagues in community settings, social care services and the independent sector.

  • Partnership working across organisational boundaries to secure a seamless journey along the stroke pathway for stroke survivors and their carers.

  • Public awareness of risk factors leading to stroke.

  • A link to the North Somerset branch of the Stroke Association.


Who is the service for?

People with a confirmed diagnosis of Transient Ischaemic Attack (TIA) or stroke who are registered with a North Somerset GP and are within 2 years of their stroke date.
