What is the service?
The Stroke Care Co-ordinator provides a community based specialist stroke service with:
- Patient centred holistic assessments, advice, information
and support to self care.Referral to and liaison with
other relevant services.

Stroke education and training support to colleagues in community settings, social care services and the independent sector.

Partnership working across organisational boundaries to secure a seamless journey along the stroke pathway for stroke survivors and their carers.

Public awareness of risk factors leading to stroke.

A link to the North Somerset branch of the Stroke Association.
Who is the service for?
People with a confirmed diagnosis of Transient Ischaemic Attack (TIA) or stroke who are registered with a North Somerset GP and are within 2 years of their stroke date.
