Multiple Sclerosis Specialist Nurse
What is the service?
The MS specialist nurse works clinically with people with MS who are registered with North Somerset General Practitioners (GPs). This service co-ordinates care and provides input in a timely manner to people with MS ensuring that they can access support when it is needed. The aim is to work in partnership with patients and carers/family to empower them to self-manage and become experts in their own condition. The MS nurse works with neurologists, GPs, NHS community staff, social services, voluntary agencies and charities to ensure holistic care is provided to patients who have complex care needs and have the potential to become high intensity users of health care. The MS nurse is additionally required to provide education programmes to both health and social care professionals.
The MS Specialist Nurse provides:-
- nursing, clinical and social assessment of patients with MS
- a nurse led clinic in collaboration with the Community Services Continence Service and MS Centre Physiotherapy Service
- education of health and social care professionals
- prompt treatment of relapse/crisis intervention
- management of relapse/exacerbation
- prevention of hospital admission
- facilitation of treatments within the community e.g. steroid therapy, IV antibiotic therapy
- promotion of early, safe, coordinated discharge incorporating intermediate care as/when necessary.
