Community In Reach

This service consists of a team of 3 nurses working flexibly to facilitate discharge of North Somerset patients from Acute Hospitals in Bristol.

The expectation is that the team will discharge patients to the community or re-ablement teams as soon as possible.

This service does not operate at Weston General Hospital as they have their own internal discharge team.
